Methods

Narrative review. The authors summarize then-current (through September 2020) molecular accounts of SARS-CoV-2 entry and pathogenesis — ACE2, TMPRSS2, cathepsin L, CD147, cytokine-release hypotheses, and coagulopathy — and survey drug classes under discussion at the time (antimalarials, RAS blockers, protease inhibitors, polymerase inhibitors, IL-6 blockade, melatonin). They then compile published peptides and thymic preparations that, in prior literature, had been proposed to inhibit viral proteins, modulate immunity, or support bronchial epithelium. No new patient cohort, no new in-vitro antiviral assay, and no efficacy endpoint is generated by this article itself.

Findings

The authors group candidate peptides into three research buckets: fragments aimed at viral proteins (for example nsp10-derived sequences and spike heptad-repeat peptides HR1-1 and HR2-18, cited from earlier SARS-CoV work); immunomodulators, which they call the most promising group at low concentration — thymopentin (RKDVY), Thymogen (EW), Vilon (KE), Epitalon (AEDG), and the calf-thymus polypeptide complex Thymalin; and bronchoprotective sequences EDG (Chonluten) and AEDL (Bronchogen). They argue, from that prior work, that short immunomodulatory peptides might help restore cytokine balance and that bronchopulmonary peptides might be studied as adjuncts. They recount one published clinical observation of Thymalin added to standard care in a single severe COVID-19 case (Lukyanov, Kuznik, and Khavinson, 2020) — a case report in another journal, not a trial run here.
